THE CONSULTATION BEFORE AN INJECTION

An injection is not scheduled as a predetermined procedure. The first step is an initial medical rehabilitation consultation with imaging examination. The consultation lasts 60 minutes for 1 region or 80 minutes for 2 regions. During this appointment, Dr Augustin Dima assesses the patient's current condition, determines whether an injection is indicated and identifies the most appropriate treatment option.

During the initial medical rehabilitation consultation, Dr Augustin Dima performs the imaging examination necessary to assess the current condition of the targeted area and determine the appropriate therapeutic indication.

If the patient has recently undergone an imaging examination performed by Dr Augustin Dima, the physician may determine that the findings are still relevant and that the evaluation does not need to be repeated.

Previous investigations performed by other medical providers may be presented and reviewed during the consultation. However, they do not automatically replace the imaging examination performed by Dr Augustin Dima, as the patient's current condition must be assessed before a procedure can be recommended.

Based on the medical consultation and imaging findings, the physician determines whether an injection is indicated and which of the available options — hyaluronic acid or PRP — is appropriate for the condition being evaluated, and whether the procedure should be performed under ultrasound guidance.

During an ultrasound-guided injection, ultrasound imaging is used during the procedure to visualise the targeted area and guide the administration.

TYPES OF INJECTIONS

Hyaluronic acid and PRP injections are distinct procedures with different sources and characteristics. Selection is not based solely on the name of the substance or product. Following the initial medical rehabilitation consultation with imaging examination, Dr Augustin Dima determines whether an injection is indicated, which option is appropriate and whether ultrasound guidance is required.

HYALURONIC ACID INJECTIONS

Hyaluronic acid is naturally present in synovial fluid and contributes to joint lubrication and cushioning during movement. Its intra-articular administration, also known as viscosupplementation, may be included in a treatment plan to support mobility and reduce discomfort when medically indicated.

Response to the procedure varies from one patient to another. A hyaluronic acid injection does not rebuild cartilage or cure osteoarthritis.

ABOUT THE PRODUCTS USED

For hyaluronic acid injections, it is important to understand that the available products are not identical.

The clinic uses single-dose products with a 12-month administration interval specified in the product information. The range includes formulations containing 90 mg of hyaluronic acid in 3 ml, with molecular weights of up to 5,500–6,000 kDa after sterilisation, as well as cross-linked formulations.

These characteristics do not establish a universal hierarchy among products. Selection is not based on the principle that "the highest concentration is the best"; the physician chooses the product according to the medical indication and the individual characteristics of each patient.

The administration interval stated in the product information does not guarantee the duration of the clinical outcome.

PLATELET-RICH PLASMA (PRP) INJECTIONS

PRP is prepared from a small amount of the patient's own blood. The blood is processed by centrifugation, and the platelet-rich plasma is then injected into the target area.

Platelets contain growth factors involved in the body's natural tissue-repair processes. The procedure may be included in a treatment plan for selected musculoskeletal conditions only after medical evaluation.

Response to the procedure varies from one patient to another, and a PRP injection does not guarantee healing.

WHEN AN INJECTION MAY BE CONSIDERED

The indication for an injection may be assessed when the symptoms and diagnosis suggest that an injectable procedure could form part of the treatment plan. The presence of any of the situations below does not automatically mean that an injection is recommended.

JOINT CONDITIONS AND SYMPTOMS

The indication may be assessed in situations such as:

  • joint pain occurring while walking, climbing stairs or carrying out daily activities;
  • joint stiffness;
  • reduced joint mobility;
  • discomfort associated with mild or moderate osteoarthritis, particularly of the knee;
  • symptoms that persist despite conservative treatment, such as therapeutic exercise, medical rehabilitation or medication prescribed by a physician.

TENDONS, OVERUSE AND POST-TRAUMATIC RECOVERY

The indication may also be assessed for selected musculoskeletal conditions, such as:

  • persistent tendon pain;
  • overuse injuries;
  • certain tendinopathies;
  • recovery following trauma, depending on the individual case;
  • musculoskeletal discomfort that has not improved sufficiently with conservative treatment.

The assessment takes into account the diagnosis, stage of the condition, age, activity level, medical history, previous treatments and investigation results.

Following the consultation, the physician may recommend a hyaluronic acid injection, a PRP injection, another treatment option or may determine that an injection is not indicated.
